  • The cost of services varies depending on your insurance coverage.
    For self-pay patients, please contact Clear Hope Psychiatry directly to obtain pricing information.

  • All new patient intake forms and consents must be completed at least 24 hours prior to your appointment

  • Appointments may need to be rescheduled if required documentation is not submitted on time
